Measurements for ordering the Waist Chastity Belt Measuring Kit

You can order it by fax or letter with the Waist Chastity Belt Measuring Kit Order Form, and we will directly mail (when prepaid) the Chastity Belt Measuring Kit together with instructions and Instruction Videos on CD to your address. What we need, however, are the following roughly taken measurements (please fill in):
